Why is this medication prescribed?

Tretinoin is used to treat acne. It promotes peeling of affected skin areas and unclogs pores. Tretinoin controls acne but does not cure it.

This medication is sometimes prescribed for other uses; ask your doctor or pharmacist for more information.

One of the tretinoin creams is used to treat fine wrinkles, dark spots, or rough skin on the face caused by damaging rays of the sun. It works by lightening the skin, replacing older skin with newer skin, and by slowing down the way the body removes skin cells that may have been harmed by the sun. Tretinoin works best when used within a skin care program that includes protecting the treated skin from the sun. However, it does not completely or permanently erase these skin problems or greatly improve more obvious changes in the skin, such as deep wrinkles caused by sun or the natural aging process.

Tretinoin is a derivative of vitamin A. It is used on the skin (topically) in the treatment of mild to moderate acne and on skin that has been damaged by excessive exposure to the sun. Tretinoin irritates the skin and causes the cells of the skin to grow (divide) and die more rapidly, increasing the turnover of cells. The number of layers of cells in the skin actually is reduced. In patients with acne, new cells replace the cells of existing pimples, and the rapid turnover of cells prevents new pimples from forming. By a similar mechanism, tretinoin can reduce some wrinkles, areas of darkened skin (hyperpigmentation), and rough areas of skin, all of which occur in sun-damaged skin. In patients with sun-damaged skin, improvements in the skin usually are seen within the first 3 to 4 weeks of treatment. Brown spots begin to fade after six to eight weeks. Wrinkles decrease or disappear after three to six months. Following application to the skin, a minimal amount of drug is absorbed into the body. The FDA approved topical tretinoin in 1971.

How should this medicine be used?

Tretinoin comes in topical liquid, cream, and gel. Tretinoin usually is used daily at bedtime or once every 2 or 3 days. Follow the directions on your prescription label carefully, and ask your doctor or pharmacist to explain any part you do not understand. Use tretinoin exactly as directed. Do not use more or less of it or use it more often than prescribed by your doctor.

Your acne probably will get worse (red, scaling skin and an increase in acne sores) during the first 7-10 days that you use this medication. Nevertheless, continue to use it; the acne sores should disappear. Usually 2-3 weeks (and sometimes more than 6 weeks) of regular use of tretinoin is required before improvement is seen.

Use only nonmedicated cosmetics on cleansed skin. Do not use topical preparations with a lot of alcohol, menthol, spices, or lime (e.g., shaving lotions, astringents, and perfumes); they can sting your skin, especially when you first use tretinoin.

Do not use any other topical medications, especially benzoyl peroxide, salicylic acid (wart remover), and dandruff shampoos containing sulfur or resorcinol unless your doctor directs you to do so. If you have used any of these topical medications recently, ask your doctor if you should wait before using tretinoin.

Initiating remission for a certain type of acute promyelocytic leukemia (APL) that has failed to respond to other therapies. It may also be used for other conditions as determined by your doctor.

Tretinoin is a retinoid. How it works in APL is not completely understood. It is thought to decrease the growth of cells associated with APL.


Skin use

Tretinoin is most commonly used to treat acne. It is also used off-label to treat and reduce the appearance of stretch marks by increasing collagen production in the dermis.

In topical form, this drug is pregnancy category C and should not be used by pregnant women.

People using the topical form should not also use any cream or lotion that has a strong drying effect, contains alcohol, astringents, spices, lime, sulfur, resorcinol, or aspirin, as these may interact with tretinoin or exacerbate its side effects.

Leukemia

Tretinoin is used to induce remission in people with acute promyelocytic leukemiawho have a mutation (the t(15;17) translocation 160 and/or the presence of the PML/RARα gene) and who don't respond to anthracyclines or can't take that class of drug. It is not used for maintenance therapy.[9][10][11]

In oral form, this drug is pregnancy category D and should not be used by pregnant women as it may harm the fetus.
